W, Taylor Ridge, IL 61284 (309-787-3351).  When you register, please include: name, address, phone number, conference (Salem is in the NE Conference), church name/address, any special needs (diet, physical, etc), and email.  Upon receiving registration a confirmation letter including a map will be sent to you.
